What's the voltage (VAC), power frequency (HZ) at application place, What's kind of load, Power (kWt), Current(A), MLFB number, if available, Upload photograph where is shown the terminal where you connected 3-phase cable? At first sight I would assume that the line filter was damaged. This may be caused by illegal line supply conditions.

Siemens service personal claiming that, 1) Due to design issue. 2) As previous post mentioned that supply issue. We ensured supply within limits using power analyzer. Fortunately our drives cover under AMC, all were replaced by Siemens.
