I'm confused as to how an IEC timer should function given this particular use case.

IEC timer is defined in an array structure in a global DB and used in an FC.

Enable is on and the timer does nothing.

What am I missing?

Note: Using a 1510SP-F CPU in TIA Portal V19 Unified.

According to the NOTE and documentation and 100's of forum topics very similar to your own, without a Timer.Q evaluation somewhere in the plc logic, the timer never checks to see if the Q output needs to be turned on.

Add a timer.Q NO contact somewhere and it will evaluate the timer status and either turn on the Q output or not.

Also, according to Siemens S7-1200/1500 Programming and Style Guide, it is bad to directly access instance tags outside the actual block.

DB500....Enable on the Timer's IN... is a case of that.

That did it.  Never noticed that before as I usually use the done bit.  Because, you know...it's a TIMER.

In this case though all I really need is the elapsed time.
