This seems to be ok, but what about the other +V and 0V wire? Check the 12V output is not overloaded. Make shure A and B show more than 12V (against ground and 0V) as high signal. (You can use a normal (digital) voltage meter, just turn the encoder slowly until you have a high signal) According to the attachment, I'm not shure the encoders "XCC 15......Y" give enough voltage to the outputs (additional note: RS422 - 5 V.)
