Safety OUTPUT overtemperature

I have the following problem on my Safety output card F-DQ 4x24VDC/2A PM HF (4-FDQ 6ES7 136-6DB00-0CA0):

At this moment i have the following diagnostics on my output card when i insert it to its base module in myIM 155-6 PN STmodule:(Picture in attachement)

Diagnistic status: "Overtemperature","the output stage is overloaded and gets too hot", Solution: "match load and output stage": (Picture in attachement)

Chnnel diagnostics: FI/O Passivated (Picture in attachement)

This fault comes with or without output wires connected to output channel. And Sins i only need one of the outputs i only activated channel 0, with light test and wire break diagnosis:(Picture in attachement)

The overtemperature fault goes away as soon as i disconnect the power supply and Profinet cable. But FI/O remains passivated and couldn't be reset!

Does anyone have any idea what could be the cause and if it can be fixed somehow?

my own assessment is that the output card is damaged and needs to be replaced, But i have to make sure before i order a new one.

have you tried another channel? 

and also try to swap the module position on the rack configuration.

Error =  Overtemperature, Match load and output stage.

We encountered the same issue while replacing the module and its base which led us to rotate the white plastic key and key hole between the components. In doing so, the black pin connector also rotated which altered the pinout which could cause a short and thus the error.  

Rotating the key and black pin connector to its original orientation resolved the issue. In our case, the semi-circle on the black pin connector was facing down.
