Significance of A031 = Speed controller monitoring has activated i.e. difference between set point & actual speed is much more than the values set in parameter.In you case speed difference is more than 3% of rated rpm for longer time than 3 sec.

Please check what type of speed feedback device you have. Please check the coupling between motor & speed feed back device.

If it is analog tacho, check its carbon brushes, ripple containts in outputs. If it is encoder, please check signal voltage level, it shielding etc. etc.

You do not need to change any parameter i.e. 388 nor 390 no any indices of it.

From the attached snap of tacho commutator, carbon deposition seems to be more on commutator. Clean carbon deposition on the complete commutator periphery by fine polish paper by rotating tacho.

Check the resistance of tacho at both leads. Measure resistance by rotating tacho at different angles. Resistance value must be same (+/- 5% acceptable) If it differs more, then you need to replace tacho.

Another best & correct way to check tacho is, rotate tacho at known speed & measure tacho voltage out put on oscilloscope. This way you can find out ripple content in output. If ripple is more than 10%, replace tacho.
