Can you help me to write one line of POLY command with this data set?

I couldn't solve exactly how 3rd degree POLY command works, i will be very glad if you help me to write only 1 line from this data set. The values are also available in excel format in the attachments. C axis is the rotary axis around Z axis, and V is the same as X. It is also in the attachment. Thanks,
