Hello putta, It seems clearlyy that there is more than nominal voltage (120/230)VAC was applied to these channels. I do not know what kind of protection you did with this module. Please clarify. Also may be one of strong possibilities that in some cases Ground is linked to Neutral. And in case of phase to ground fault, there will be probability of failure for this module. Thus because in this you have for example digital inputs comming from Phase 1.
If this is your case, I recommend to use an isolating transformer to ensure that neutral of the module is different than neutral of your system or its ground. Hope this helps.

Dear sir, Thank your for your information!. Iam talking about our safety protection as earlier qoute, we using one isolation transfermer and MCB protection at terminal end also MOV ( surge resistor coils).
