Where i should connect my GND into inputs

Hello,

i've got a question. I'm making a project for my final exam in logo soft control v.7.1 . And i've made circuit which consists of amplifier with pt100 (attachment with figure). Output of the board is 0 - 10 V (means 0 °C - 300 °C). But i would like to display this number in plc and add some timers and other staff.

And i'm asking for help, my board has 2 Outputs and i dont know where they should be connected to analog inputs at Logo . I need to have these 2 together, when i use to measure voltage of the output i need to wires but in conquery i dont see any solution of my problem. 

The temp is changing because of the RTD senzor resistence. But i cannot represent that at plc. Because of 2 Outputs of the board.

Hello i tried it today, it actully measure temp, but in some way i dont understand. Sometimes it show true temp (approx. 110 °C) but in few seconds both temps goes out of nowwhere to 150 °C End immediately down to 0°C - starts again increase to actual temp and then whole process repeat. Any ideas how to make it meassure actual temp whole time ?

I could be very helpful.

Wires are under the terminals without insulation.
Screws are properly tightened.
