Digital Input not updating tag table

Having an issue that might be due to a couple of items.

Problem:
Wired a new digital input signal to an existing input module that has existing channels configured and are working.  Confirmed the sensor is functioning, sending a signal, and the input module is sensing that signal via the LED for the channel lighting up.  Configured the channel in the device hardware configuration and confirmed it is in the tag table.  Issues start when I then download to the PLC.

I am not certain if this is the issue but is my current theory.  The tag table has both fail-safe tags and normal tags intermixed, because of this when making a change it is requiring access to a fail-safe object.  I don't have the fail-safe password, already reached out to our equipment vendor for it, so I press cancel.  The output console shows the changes were loaded but that it failed due to an abort.  However it seems like the changes are present and an online/offline comparison shows they are present.  

After the maybe failed maybe didn't download triggering the sensor does not update the PLC tag.
